Please see the samples of each required document below. You can download a blank W-9 form and direct deposit form. Your Certificate of Insurance (COI) must list LUXY Technologies Inc. as both the certificate holder and an Additional Insured. The vehicle(s) covered under your policy must also be listed in the Description of Operations section. This section must explicitly confirm that LUXY is included as an Additional Insured for general liability coverage. Once your documents are ready, upload your updated COI directly to your partner account. LUXY will then contact your insurance agent to verify your coverage. All submitted documents must be in PDF format.
